Education, Health and Care Plan - Parent Survey

Overview

This survey is to ask parents about their experiences of applying for an Education, Health and Care plan for their son or daughter. (Referred to as an EHC Plan) It will be used after each plan has been approved and will be used to identify areas for improvement around the process in Northamptonshire. The survey is anonymous. It should take around 10 minutes to complete.

This forms part of the SEND strategy to monitor effectiveness.
